Rental Program Packages

In order to receive package pricing, a six month contract is required. Participating in the six month program requires either two or four rental days per month. The total monthly cost of the rental is due on the first day of each month. In order to guarantee the device, Phoenix Medical requires the rental days be booked at the time of contract signing. If, in the event the facility needs to reschedule any or all of the rental dates, it must be done at least 48 hours prior to the originally scheduled date.

Package A includes rental of the same device two times per month for six consecutive months.

* Standard Harmony Rental includes three AFT modules: HR, VL/PL, SR. You may substitute one of the specialty modules for all three AFT's.

Cancellation Policy: Appointments must be canceled at least two (2) business days prior in order to avoid a fee of $250 per laser.
